Please register to access your contact lens account online. Your information must match the information that we have for you on file in our office in order to authorize your account.
After submitting this form you will be returned automatically to the home page. Please allow 1 to 2 business days for your registration to be processed.
After 1 to 2 business days, you will receive an email with your account name and temporary password.
Specialty contact lenses including Rigid Gas Permeable lenses, Hybrid Gas Permeable lenses, Scleral lenses, Corneal Refractive Therapy (CRT) lenses, and Kerasoft lenses must be ordered in office.
